Norwich’s Integrated Anticipatory Care Team
INTERACT supports people whose housing or home environment is negatively impacting their health and wellbeing.
INTERACT is a multi-agency team with members from Age UK, Norfolk Citizens Advice, Norwich City Council and Norfolk County Council. The team works in partnership with the Home Improvement Team at Norwich City Council.
How can INTERACT help?
- Creating an action plan to help the person achieve their goals
- Adapt their home and environment so it is suitable for their needs
- Offering practical support to help with moving home
- Providing assistance with benefits entitlement and claims
- Giving guidance and support to get involved with their local community
- Arranging help with decluttering and organising
We welcome referrals from all local system partners via www.norwich.gov.uk/healthyhomes
or through your local Integrated Care Coordinator. Please ensure that you have discussed the referral with your client first.
Who can we help?
People 18+ whose health and/or wellbeing are adversely affected by their housing situation or home environment, who are registered with a Norwich GP and are experiencing one or more of the following:
- Frailty
- Two or more serious health conditions
- Complex needs
We are unable to support:
- People in temporary accommodation or permanent residential care settings
- People in crisis (e.g. urgent physical or mental health issues)
- People with a score of 7+ on the clutter image scale. Those scoring 1-3 will be accepted. Those scoring 4-6 will be considered on an individual basis
